Internship documents are attached.

Documents that must be submitted to student affairs during the internship application;

1-Internship Acceptance Form

2-Internship Agreement

3-Photocopy of the agency document

4- Occupational Health and Safety Certificate

5-A document showing your health insurance status (Health Provision) that you can obtain from e-government or SSI.

You will submit all other documents within the dates announced by student affairs when you successfully complete your internship.
